Report Structural Barriers and Suggest Ways to Improve

ESA takes the equitable inclusion and safe participation of all members of our community seriously. This form was created so that members of our community can report structural barriers they observe to leadership or make a suggestion for how we can better create a safe and welcome environment in ESA.

If you observed or were the victim of bias or felt unwelcome at an ESA event, entity or program, you can anonymously report that incident through our Code of Conduct for Events.

How to Use This Form

Select from among the first two options to have more questions appear. If you find that you don’t have a response for a required question, “I don’t know” or “I’m not sure” is a suitable response.

What Types of Incidents or Suggestions Can I Report or Submit?

You can submit for any of the following:

  • A structural barrier — An ESA policy or practice that blocks equitable and inclusive participation of diverse people in the Society’s activities or programs, or that hinders justice
  • A suggestion — any ideas that advance diversity, equity, inclusion and justice in ESA

Is This Submission Anonymous?

It is unless you would like ESA to follow up with you — adding your name and/or contact information to the submission is completely optional, and no location or IP data is preserved.

What Happens After I Submit?

When you submit a report, an email is sent to ESA’s designated receivers: the executive director and chief financial officer. Depending on the nature of your submission, your report could be referred to the ESA president, an appropriate committee or a special task force for appropriate action. You may be contacted for more detail if you included your contact information in your submission; otherwise, your anonymity is guaranteed.
